SPS Title IX/Sexual Misconduct Reporting Form

The Shepherd Public Schools Sexual Harassment Policy prohibits sexual harassment including sexual assault (rape, fondling, incest, and statutory rape), dating violence, domestic violence, and stalking.

By filling out this form, you are notifying the Title IX Coordinator of an initial complaint of sexual harassment, sexual assault (rape, fondling, incest, and statutory rape), dating violence, domestic violence, and/or stalking. This is NOT a formal complaint and may not initiate an investigation. Upon receipt of this report, the complainant (the person alleged to have experienced the conduct) will be contacted by email with a letter that includes Title IX policy information, resources, and an invitation to meet with the Title IX Coordinator. The complainant will be presented with information and the choices available, including supportive measures as appropriate.

Conduct may be addressed through the Title IX Sexual Harassment Policy. This will be determined based on the status of parties and jurisdiction.

You may report anonymously by omitting information in any field. Only fields marked with (required) must be complete for the form to be submitted. Please note anonymous reports may be difficult to investigate.

This form does not constitute filing a criminal complaint with the police. To file an official police report, please contact the Isabella County Sheriff's office at 989-772-5911.
