Complete the questionnaire as truthful as you can. Employees can respond to statements on a scale from 1 (Strongly Disagree) to 5 (Strongly Agree).

Instructions for Scoring and Analysis

  • 100-125 overall: Indicates a very high level of overall psychological safety. The workplace is extremely supportive and psychologically safe.

  • 75-99 overall: Indicates a moderate level of overall psychological safety. The workplace has several strengths but also notable areas for improvement.

  • Below 75 overall: Indicates a low level of overall psychological safety. There are many areas that require immediate attention and improvement.
