This application is being conducted to determine the eligibility of your 4-year old for the North Carolina Pre-Kindergarten and/or Head Start Programs offered in Craven County. Accurate information is needed from you that will remain confidential. This information will become a part of the NC Pre-K, Title I, and/or Head Start databases and will be used to determine eligibility of the child and monitor their progress throughout the year. You will be asked to electronically sign a release of information at the end of this interview to participate.
