申請流程：

請先準備申請入會所需文件之電子檔，網上填妥入會申請及上傳文件，待理事會開會審批入會申請。通過審批後會通知申請者繳交入會費用，本會確認繳費完成並回覆會籍正式生效。

溫馨提示：請先預備以下入會所需文件之電子檔，再填寫入會資料。

1. 證件相 (圖片檔)

2. 身分證明文件，證件正反兩面 (圖片檔或PDF檔)

3. 物理治療學位證書 (PDF檔)

4. 如有其他學歷認可證書 (PDF檔)

Application process

Please prepare the electronic file of the documents required for membership, fill in the application online and upload the documents, and then wait for the executive board to approve the application. After the approval, the applicant will be notified to pay the membership fee. We will confirm that the payment is completed and reply that the membership formally take effect.

Enclosures required, please prepare the following electronic files before applying.

1. Passport size photo, image file.

2. Identification,  both sides of the ID Card or Passport, image file or PDF file.

3. Physical Therapy Degree Certificate, PDF file.

4. Other certificate issued by Support Office for Higher Education, PDF file.
