Adoption/Foster Application/Agreement

The Pulaski County Humane Society’s adoption fee includes the following:
* Age-appropriate shots
* Testing for worms, and treatment if the test is positive
* Cats tested for FIV and feline leukemia
* Dogs tested for heartworm if at least 6 months of age
* Spayed/neutered at the appropriate age
* Micro chipping at spay/neuter

If you choose to foster a pet, the Somerset-Pulaski County Humane Society will pay ALL expenses directely related to our animal(s) in your care.

Please Note: This form will take approximately 15 - 20 minutes to complete. Please plan accordingly. It also requires consistent internet connectivity.
