The FASSV-PTA organizes a centralized gift collection for teachers and staff twice per school year. Participation is completely voluntary. If you would like to contribute to this centralized gift collection, please complete the form below. The teachers and school staff will then be given an opportunity to choose the gift card(s) of their choice amongst 600+ SCRIP card options.
To ensure that the gift cards are ready on time, the DEADLINE for contributions is Wednesday May 31st by 5pm.
How-to: It's very simple, just one form per family! Please indicate the amount you would like to contribute next to the name of the person you'd like to contribute to. For the school administration staff you can also select 'Split between staff' and put a single amount and your contribution will be split amongst those staff members, or you can put an amount next to each staff name and we will attribute accordingly.
Payment can be made by check (made out to FASSV-PTA) or PayPal.
Due to a change in PayPal fee structures that occurred over the summer, the PTA can no longer accept payments via PayPal without incurring a fee. However, if for convenience, you would still prefer to pay by PayPal, you may do so provided you are willing to pay the (nominal) associated fee.
