Kindly read Section A carefully before completing Section B of the form.  The following documents should be submitted with the application form:  
a.  Certified passport-size photograph
b.  Character reference completed by a Justice of the Peace.
Please note that the application process is not complete until you have submitted all the required  documents and obtained the requisite approval from the Mentorship Secretariat.
