Confirmation Registration Fall 2023

Welcome to all students and parents/guardians in the St. Pius X Confirmation Program!

To complete registration for the Sacrament of Confirmation, fill out the form below and provide a copy of the candidate’s baptism certificate. If you don’t have a copy of the Candidate’s Baptism certificate, please see http://stpiusxchurch.org/candidate-baptism-certificate/.
*When registering you will have to upload your candidate's baptism certificate. The registration form cannot be submitted without it.

OUT OF PARISH Candidates must also provide a letter of permission to receive the sacrament of Confirmation at St. Pius from your current church pastor. Letters can be submitted the same way Baptism certificates are submitted (see above).
