Formfacade currently supports only public forms. If you have enabled "Restrict to users in your domain and its trusted organizations" option, then it will not work.

The file upload question in Google Form requires respondent to signin with their gmail account to upload files and submit the form. This form will not be publicly visible. Users will have to signin with their gmail accounts to view the form, upload the files and submit the response. You can use the Formfacade add-on to convert Google Forms file upload question into HTML file upload and let your users upload files without sign in.
