SCHOOL PARTICIPATION FORM

To be completed and signed by the Head of School of the participating school and returned to

1. We (the participating school) will be participating in the 2025 Columbus Baseball Invitational (CBI 2025).

2. We hereby assume the primary responsibility for the conduct, health, and safety of all of our school’s participants, including but not limited to our baseball players, coaches, chaperones, etc.

3. We hereby affirm that in the event any damage or loss is caused by any of our participants, requiring repair or replacement of the damaged or lost item(s), our school will assume the cost for repairing or replacing the item(s).

4. We will pay the per-team entry fee of $6500.00, which includes a non-refundable deposit of $500, for which we will receive housing, meals, and participation in CBI 2025 for up to 20 participants (any combination of 20 baseball players, coaches, chaperones, etc.) There may be a charge for additional participants. There is no reduced fee for under 20 participants. The per-team entry fee can be reduced by $1500 through the purchase or sale of $1500 in ads that will be featured on the 2025 CBI website. All ads must be secured by April 1, 2025.

5. For each and every participant for whom we have paid the applicable fee, we will receive appropriate housing on May 18-20, and Kosher meals: lunch and dinner on May 18; breakfast, lunch, and dinner on May 19, and breakfast and lunch on May 20, 2025.

6. CBI is an outdoor event and the potential for rain may hamper or affect the ability to complete all tournament games. We understand that, in the event of inclement weather, the tournament organizers will make any adjustments to maximize tournament play.

7. We agree to make travel arrangements that will have team arrive in time for Tournament play on Sunday beginning at approximately 3 pm and ending on Tuesday at 3 pm.

8. We understand that after April 1, there are no refunds or reductions in tournament fees for any reason.

9.
